So far the only thing I’ve noticed is a little clipping around 12:30 while Asakura is disappearing. Otherwise it seems almost perfect up until then.
I’ve finished watching the episode now, and everything else seems perfect as well.
At 02:11 the text “Ultra Director Haruhi Suzumiya” is barely visible against Haruhi’s white sleeve. Haven’t watched any further yet.
I’m not 100% sure about this, because English isn’t my first language, but from 04:32 to 04:40:
“If Asahina-san found out that unbecoming pictures of her maid costume…
…in knockout poses…
…have been spread around the world…
…she’ll definitely faint on the spot.”
Shouldn’t it be either:
1.) “she’d definitely faint on the spot.” (conditional)
OR
2.) “If Asahina-san finds out…” (future simple)
I didn’t find anything else wrong with the episode.
All of the English text in the opening translating the opening credits are too faint. Don’t blur them. Make them solid characters and put a drop shadow behind them like you did for the title of the episode.
01:04 – “You could even call it a date.” “Kill two birds with one stone.” Make this into one whole line not two separate ones since The first line dissappears too quickly. I can read that fast but I doubt others can.
01:54 – “I wonder if some kind of full-blown trouble will happen…” I’m just commenting on this from the english dub. She doesn’t wonder she wants. So change it to “I want some kind of full-blown trouble to happen…”
I know this is QC and all but I see pixleation on solid colors like at 02:25 where the white on Haruhi’s uniform pixleates.
02:56 – “After School when everyone’s gone Come to classroom 1-5″ Lowercase the “C” on come or make it two separate sentences.
04:32 – “If Asahina-san found out that unbecoming pictures of her maid costume…” Like Vertend said but change it so something like “If Asahina-san found out those unbecoming pictures of her in a maid costume…”
07:10 – “…the higher ups are all stick-in-the-muds who can’t keep up with change. Change “stick-in-the-muds” to “sticks-in-the-mud” Since its a referral to multiple people (sticks) in the (one) mud.
12:37-12:43 – Pixleation/Encdoding error on the entire bottom of the screen.
23:36 – “Ending Theme” translation stuff including the name of the song and people who sang it, doesn’t cover all the Japanese text. I see either white spots or partial characters.
23:54 – “Animation Production” cover up is darker than the background so you you can see the box, and it doesn’t cover all the japanese text.
Those are all the issues I had.

Regarding your tr-fix at 1:54, I do not know Japanese, so I am unwilling to change it without verification from someone who I trust actually knows Japanese.

12:37 was an error that was present in the raw, it’ll take a little while to fix so don’t expect a release anytime soon.

The animation production text will be fixed.
it looked fine to me. There are some spots on it that came up glitchy, but i think its just my computer.
Like I said for 1:54 I was going with the English dub and the subs for the DVD, and that was what she said there. Keep it, or confirm with a TLer. I just brought that up based on what the DVD had.
As for the encoding problems it might be my media player, as well as how the cover ups are shown in the ending. because this is what I see.

at 9:58, it doesn’t read well as “Why intrusion was granted”, though this is what is being said literally. It sounds better “That’s why intrusion was granted” or “…why intrusion was granted” to indicate that is is continuing the train of thought from the previous sentence. right now, it reads awkwardly.
at 10:18, same problem (not translation, but readability), though I don’t know how to make “Go ahead and try?” sound better…it’s so awkward, but the best I can think of is “Why don’t you try?”. this isn’t good either, though, since it doesn’t match the fact that Asakura is speaking succinctly…
as for 1:54, “I wonder when some kind of full-blown trouble will happen…” seems to fit better with the atitude here…
I do speak some Japanese and I would translate what she says at 1:54 as ‘Won’t any kind of full-blown trouble happen?’. Besides that, I was quite confused by the ellipsis at the end of the line.
I found that after the opening sequence, there was some pixellisation, and when Asakura began to disappear. Other than that, it was all ok.
Though, I did find that the first and second episodes seemed to get stuck just before the opening sequence.
